TESE OR BIOPSY
Testicular extraction of sperm involves opening the scrotum and taking a very small sample of testicular tissue, sometimes from several areas of the testicle. Sperm are then collected using a microscope to identify individual sperm. In case sperm cannot be found in one testicle, the surgeon opens the other testicle and takes samples from it.

Varicocele
Varicocele is an enlargement of the venous plexus that drains blood from the testicles. It most often occurs between the ages of 15 and 20 and is usually asymptomatic. According to size, we can divide them into small, medium and large. Although they can occur on the right and left, for anatomical reasons in almost 90 percent of cases they occur on the left. They are present in 10 to 20 percent of the general population, and in men with reduced fertility they occur in up to 40 percent of cases.
How to recognize that you have varicocele?
The exact mechanism by which varicocele impairs sperm function and production and their structure has not yet been resolved. It is thought that temperature, lack of oxygen or toxins may have an effect. Namely, the temperature in the scrotum is physiologically lower, which is important for the development of sperm, and the expansion of the venous plexus increases the temperature and thus adversely affects spermiogenesis.
The milder form of varicocele may not show symptoms, but over the years it can cause swelling and pain in the testicles due to sudden enlargement of the veins. If you stand for a long time or the weather is warmer, the blood vessels in the testicles become more visible and the discomfort increases. In more severe forms of varicocele, the testicle may change in appearance and size. Varicocele is easy to diagnose with a simple urological examination, which may include ultrasound. Often the professional occupation that individuals do can affect the appearance of Varicocele.
Can Varicocele cause conception problems?
The temperature in the scrotum, which protects the testicles, is usually a few degrees lower than body temperature, and healthy sperm require a lower temperature than normal body temperature. The increased temperature in the testicles, caused by Varicocele, negatively affects the production of sperm, as well as the quality of sperm.

The impact of varicocele on sperm quality and number is difficult to predict, as it has been observed that sometimes men with large varicocele do not have fertility problems and vice versa - significantly reduced sperm quality can be seen in barely noticeable varicocele. Therefore, treatment is usually not necessary, except in cases of impaired testicular function, testicular shrinkage, or persistent pain or discomfort. In these cases, treatment is most often surgical.
